Agreed Conditions with the Health Protection Service 1. A permanent fixed residual current device (RCD) must protect the electrical power serving all amplified music equipment used for the purposes of live music. 2. A Building Regulation Completion Certificate shall be submitted to the responsible authority for public safety within a reasonable time period after the premises has opened for business. 3. The applicant, or their representative shall consult with the Health Protection Service in order to ensure that relevant works, with respect to public safety, are completed to the satisfaction of the Health Protection Service. Agreed Conditions with the Environmental Protection Service. 4. Licensable activities shall not commence until a scheme of sound attenuation works have been carried out to the written satisfaction of Sheffield City Councils Environmental Protection Service. The scheme shall include the formation of an acoustic lobby to the customer entrance to the premises and a scheme of works to mitigate noise transmission to the upper floors of the building, including attenuation of noise from any plant or ventilation provisions installed. 8. A management plan shall be held on site detailing the importance of adhering to the agreed Smoking Policy and Dispersal Policy. Staff shall be suitably trained to implement the management plan and a copy of the plan and referenced policies shall be made available upon request by the Licensing Authority. Dispersal Policy 10. At the end of the evening management and staff will assist with the orderly and gradual dispersal of patrons. 11. Staff members (including door personnel if any) will advise patrons to leave the premises quickly and quietly out of respect of any neighbours. 12. Notices will be displayed requesting customers to leave quietly and in an orderly manner out of consideration to any neighbours and attention will be drawn to these notices by members of staff. 13. Any patrons awaiting the arrival of a taxi will be encouraged to wait inside the premises. Smoking Policy 14. The smoking area will be in range of the premises CCTV system. 15. Suitable receptacles will be provided and maintained for the disposal of cigarette litter within the area. 16. Signs will be displayed within the area requesting customers keep noise to a minimum. 17. The area will be kept clean and tidy at all times. 18. A designated member of staff shall be responsible for the smoking area when it is in use. Agreed conditions with South Yorkshire Police. 19. A "Challenge 25" policy shall be implemented and maintained at the premises. 20. A CCTV system to the minimum specification of South Yorkshire Police will be fitted, maintained and in use at all times the premises are open. CCTV images will be stored for 31 days. Police will be given access to, and copies of the images for purposes in connection with the prevention and detection of crime and disorder. 21. To acquire a suitable number of radio sets for the premises and to take the CCRAC Radio Scheme (City Centre Retails Against Crime) into use at all times when trading, and continue to be a user whilst this system is in use within Sheffield. Conditions from the operating schedule. 22. Any person left in charge of the premises must be trained in the use of any such CCTV equipment, and able to produce CCTV images to an officer from a responsible authority upon request. 23.A risk assessment shall be carried out to determine whether or not door supervisors shall be employed at the premises, and if so, the risk assessment shall specify the number of door staff to be employed and the times they shall work. 24. When employed, a register of those door staff employed shall be maintained at the premises and shall include; (i) the number of door staff on duty; (ii) the identity of each member of door staff (iii) the times the door staff are on duty. 25. The premises shall maintain an Incident log. 26. The sale of alcohol for consumption beyond the curtilege (i.e beyond any al fresco area) of the premises shall be in sealed containers only. 27. Waste collection and delivery services will not take place outside the hours of 07:00 and 21:00 hours. 28. All staff will be trained in the law regarding the sale of alcohol to underage persons. This training will be documented and repeated at six monthly intervals. 29. The venue will only accept photographic driving licences, passports, HM forces cards or a form of identification with the "PASS" hologram. 30. A refusals log to record all attempts to purchase alcohol by those under 18 shall be kept on site and all refused sales recorded. The refusals log will be made available to the Police or other Responsible Authority on request. 31. There shall be no adult entertainment or services, activities, other entertainment or matters ancillary to the use of the premises that may give rise to concern in respect of children.
