Additional Conditions
1. Persons under the age of 18 must be accompanied by a responsible adult and shall not be permitted to remain on the premises after 22:00 hours daily. 2. A suitable member of management must be assigned to act as the lead for safeguarding children and young people. This person will be responsible for risk assessments and the enforcement of the child protection policy in relation to children and shall at all times comply with the reasonable recommendations of the Sheffield Safeguarding Children Board. 3. The Challenge 25 proof of age scheme must be operated in compliance with the multi agency guidance issued by the Safeguarding Children Board, Police and Trading Sandards. 4. All employees, volunteers or entertainers must be aged 18 yeas or over. A record of each employee's name, address, national insurance number, photograph and date of birth will be retained and these details must be checked and verified by the premises manager or premises licence holder, against other recognised forms of identity (passport; photo-driving licence). These records must be retained at the premises for a minimum of 12 months from the date of employment and made available to officers of Responsible Authorities on request. 5. Signage must be displayed at points of entry that persons under the age of 18 are prohibited and that the Challenge 25 scheme operates. 6. Persons re-entering the premises must be subject to the same security and identity checks as customers first entering the premises at any given period of trade. 7. Persons using VIP/Guest passes to access the premises must be subject to the same security and age verification checks as non concessionary customers. 8. A colour CCTV system to the specification of South Yorkshire Police will be fitted, maintained and in use at all times whilst the premises are open. The CCTV images will be stored for 31 days and police and authorised officers of the council will be give access to images for purposes in connection with the prevention and detection of crime and disorder. CCTV footage shall be downloaded and provided to South Yorkshire Police on request. Members of the management team will be trained in the use of the system. 9. No amplified sound shall be played outside of the karaoke suites except through an in-house amplified sound system fitted with a sound limiter, the settings if which shall have received the prior written approval of the Environmental Protection Service. The noise limiter shall be in the control of the designated premises supervisor only. 10. No use of the first floor of the premises for the purpose of regulated entertainment shall commence without the implementation of suitable noise mitigation measures, to the satisfaction of Sheffield City Council Environmental Protection Service. 11. Regulated entertainment shall not take place on the premises other than in accordance with a noise management plan agreed in writing with Sheffield City Council Environmental Protection Service. 12. No licensable activity shall be permitted on the first floor of the premises without written consent of the Sheffield City Council Health Protection Service and Sheffield City Council Environmental Protection Service. 13. When regulated activity in the form of dancing is to take place on the premises in an area not previously approved for dancing by the authority, at least 14 days notice must be given to Environment and Regulatory Services (Health Protection Service) and the Licensing Authority, identifying the area to be used. The designated dance floor should be delineated, suitable and adequate in size for its intended use and dancing must not take place in the area until approved by the Authority.
1. No entry to new customers shall be permitted after 00:30 hours Sunday to Thursday and after 02:00 hours Fridays and Saturdays, save for pre bookings. 2. A maximum of 6 people will be permitted in the smoking area at any one time. 3. A contact number for the premises will be made available for use by the residents at all times the premises are open. 4. A written noise management plan will be agreed with the Environmental Protection Service and be made available for inspection by authorised officers of the Environment and Regulatory Services and Licensing Officers, Sheffield City Council, on request. 5. The use of door staff will be risk assessed on an ongoing basis by the Designated Premises Supervisor or a member of the management team. A risk assessment of door staff is to be made in writing and kept for 6 months and made available for inspection by authorised officers from South Yorkshire Police and Licensing Services, Sheffield City Council, on request. 6. Prominent, clear and legible notices shall be displayed at all exits, requesting the public respect the needs of local residents and to leave the premises and area quietly.
