ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
1.The premises shall maintain a digital CCTV recording system capable of recording 31 days of 24 hour recording with a monitor out of view and reach of the customers.
Images shall be retained for a period of 31 days. The equipment will be suitably secured with limited access. The CCTV system shall have coverage of all areas to which the public have access within the premises and shall include entrances and exits.
2. The CCTV system shall be maintained and operate at all times that the premises are open to the public. Images shall be made available upon request and within 48 hours to the Police or other enforcement officers in a universally accessible medium upon reasonable and lawful request by a Police officer or Local Authority officer at no cost.
3. The CCTV system will be registered and comply with the requirements under data protection. Signage shall be prominently displayed advising customers that they are being recorded on CCTV. The CCTV system will be checked on a weekly basis to ensure that it is fully operational, and a log confirming this will be kept at the premises for inspection.
4. Signage shall be displayed within the premises and shall display that a zero tolerance drugs policy is being implemented.
5. Door Supervisors shall be employed from 22:00hrs on Friday, Saturday, Sunday preceding a Bank Holiday, Christmas Eve, Boxing Day, New Year’s Eve, New Year’s Day.
6. A minimum of 2 SIA registered door staff shall be employed for up to a maximum capacity for the venue of 100 patrons. Door Staff shall remain on duty until 20 mins after closing or until all patrons have left the area, whichever occurs sooner.
7. The Premises Licence Holder shall ensure that a door staff duty register is maintained with start and end of tours of duty and will record the full 16 digit SIA licence number, full name, usual signature and on/off duty times. The register shall include name and contact details of the company supplying door staff. The register will be made available for inspection when required by any authorised officer.
8. All staff responsible for selling alcohol shall receive induction and/or refresher training (at least annually) commensurate with their role and responsibilities in relation to the sale of alcohol and the times and conditions of the premises licence. Training shall include Challenge 25, production of ID and acceptable forms of identification, the premises’ Drink Spiking Policy and Procedure, ‘Ask for Angela’, Incident Management, Policy & Procedures, Safeguarding Policy, and the requirement and process for completing both the incident log and refusal log.
9. Training shall be documented, kept at the premises for at least 12 months from the last entry, and made available to the Police and/or Local Authority upon request.
10. Alcohol for consumption off the premises shall only be provided in sealed containers.
Patrons shall not be permitted to remove drinks in open bottles/glasses or other open vessel from the licensed premises, including temporarily to smoke.
11. There shall be a register for the recording of all alcohol sale refusals, including attempted under-age sales, proxy sales and refusals to those who appear intoxicated. Details to be recorded shall include the date, time, name (if known), physical description of the person, the reason for the refusal, names of staff involved, and whether the refusal was captured on CCTV. The register shall be available for immediate inspection by any authorised officer of the responsible authorities and shall be securely retained by the licence holder for a for a period of 12 months from the date of the last entry.
12. The DPS must review the Refusals Register monthly and record the outcome within the Register, together with any remedial action taken.
13. An incident log shall be maintained on the premises to record all incidents and accidents.
Records shall include occurrences of: anti-social behaviour, admission refusals, ejections, seizure of prohibited items, casualties/unwell customers, welfare and safeguarding matters, accidents, and safety incidents.
The records shall include the date, time, and location of the incident; nature of the incident; personal details and contact information for all people involved including any witnesses, SIA and Personal Licence numbers, any crime number and details of police officers attending.
The entry must also include a note of the action taken and, where relevant, a note of the actions to prevent any reoccurrence should also be included.
14. Incident and accident records must be kept in a bound register with consecutively numbered pages or electronically on a secure digital system. In each case, the information recorded must be processed, stored, and handled in compliance with The General Data Protection Regulation. The records shall be available for inspection by any authorised officer of the responsible authorities and shall be securely retained by the licence holder for a period of 12 months after the last entry.
15. The premises licence holder shall operate the business with general consideration in respect of the neighbouring properties where clear and prominent notices will be displayed at the exits and in any outdoor public area including the smoking area requesting patrons be quiet and have consideration for neighbours in the vicinity.
16. All doors and windows shall be kept in a closed position, save for ingress and egress when any recorded or live music is being played.
17. Licensable activities shall be conducted and the facilities for licensed activities shall be designed and operated so as to prevent the transmission of audible noise or perceptible vibration through the fabric of the building or structure to adjoining properties.
18. The provision of Regulated Entertainment shall be conducted in accordance with a written Noise Management Policy, which shall be maintained on the premises and made available for inspection.
19. Deliveries to and from the premises shall be made between 07:00 and 19:00 only.
20. The Management shall provide a phone number with a local taxi company and make arrangements with that company that will agree to certain 'quiet' conditions when collecting customers at the end of the evening. These quiet conditions will ensure that all taxis vehicles will make the minimum noise possible and encourage customers to leave quickly and quietly.
21. A challenge 25 age verification scheme shall operate at the premises whereby any person who appears to be under 25 years of age, and unknown to the staff member serving as a person over 18 years of age, shall not be served alcohol unless they provide identification to prove they are over 18 years of age.
22. Acceptable forms of identification will be a valid passport, a valid photo ID driving license or a valid proof of age scheme card with the PASS approved hologram.
23. Appropriate signage advertising the operation of the Challenge 25 scheme must be displayed in the vicinity of all points of sale for alcohol.

24. Persons under 18 years of age shall not be permitted on the premises after 21:00 hours unless accompanied by a responsible adult and attending a pre-booked family-type function or dining arrangement.
i. All those under 18 to have left the premise by 00.30 hours on any day.
ii. A booking form must be used for all events and private functions.
iii. The booking form must set out clear conditions for the customer and include the printed name and signature of the venue manager authorising the booking.
iv. It must also include the name, contact details, and signature of the customer responsible for the booking.
v. Records must be retained for a minimum of six months and made available for inspection by authorised officers upon request.
25. Persons under the age of 18 shall not be permitted to make bookings.
i. Age and identification checks must be undertaken at the time of booking, and a written record of these checks maintained.
26. The Designated Premises Supervisor, or another nominated responsible person, must be assigned to act as the safeguarding lead / coordinator for the premises. This individual must comply with the guidance and training provided by the Sheffield Children Safeguarding Partnership

27. The CCTV footage will be conrolled and kept in a secure environment to prevent tampering or unauthorised viewing. A record will be kept of who has accessed the system, the reason why and when.
28. A security register for details shall be retained for a minimum of 12 months.
29. Door supervisors will be employed from 22:00hrs on Friday, Saturday, Sunday preceding a bank holiday, Christmas Eve, Boxing Day, New Year's Eve and New Year's Day. The requirement for such security at any other time to be risk assessed by the Designated Premises Supervisor or member of the management team.
30. The premises shall operate in accordance with a written Search Policy. The policy must set out the procedures for conducting searches of customers as a condition of entry or re-entry, including the use of security staff to carry out such searches.
31. All staff shall be trained in the contents and operation of the Search Policy, and records of such training shall be kept at the premises and made available to authorised officers on request. The Search Policy shall be implemented at all times when security staff are deployed.
32. A booking form must be used for all events and private functions. The form must set out the booking conditions for the customer and include the signature and printed name of the venue manager authorising the booking.
33. The form must also include the name, contact details and signature of the customer who is responsible for making the booking. Such records will be retained for 6 months and made available for inspection of the authorities.
34. The external area of the premises shall not contain any tables, chairs, or other furniture. No drinks, whether in open or sealed containers, shall be taken into or consumed within the external area. The external area shall be used for smoking purposes only.
35. No under 18’s will be permitted on the premise at any time

36. A permanent fixed Residual Current Device (RCD) must protect the electrical supply serving all amplified music equipment.
37. A satisfactory Electrical Installation Condition Report (EICR), or a combination of an Electrical Installation Certificate (EIC) and EICR, must be provided to the responsible authority for public safety within a reasonable time after opening.
38. A Building Regulation Completion Certificate must be submitted to the responsible authority for public safety within a reasonable time after opening.

39. All doors and windows shall be closed, save for access and egress of the general public and/ or than in case of emergency, when amplified sound or live music is present within the premises.
40. Amplified sound or live music shall only be played within the building and in such a way that noise breakout to the street does not exceed the prevailing ambient noise level by more than 3dB when measured;
a) as a 15 minute LAeq, and;
b) at any one third octave band centre frequency as a 15 minute LZeq.
41. No amplified sound or live music shall be played within the external area, nor shall loudspeakers be fixed externally outside the building at any time.
42. The DPS or designated member of staff must take a proactive approach to noise control, checking outside the premises to ensure that noise is kept to a reasonable level from patrons using the external area and access/ egress.
43. The premises licence holder shall prominently display A5 notices at all exits reminding patrons to leave the premises in a quiet and orderly fashion to respect the local neighbour’s needs.
