Additional Conditions
1. There shall be no adult entertainment or services, activities, other entertainment or matters ancillary to the use of the premises that may give rise to concern in respect of children. 2. Persons under the age of 18 years are prohibited unless attending a pre-booked family orientated function whereby the premises is closed to members of the general public. 3. Adult entertainment and the promotion of this is prohibited while persons under the age of 18 years are at the premises. 4. Amplified sound or live music shall only be played within the building in such a way that noise breakout to the street does not exceed: (i) background noise levels by more than 3 dBA when measured as a 15 minute LAeq (ii) any octave band centre frequency by more than 3 dB when measured as a 15 min Leq. 5. The external and inner acoustic lobby doors serving the main entrance on Eyre Street shall not be left propped open at any time. 6. A satisfactory electrical certificate should be provided for the premises before any licensable activities are carried out on the premises. 7. A permanent fixed residual current device (RCD) must protect the electrical power supply serving all amplified music equipment used for the purpose of live music. 8. A Building Regulation Completion Certificate shall be submitted to the Public Safety Responsible Authority. 9. A colour CCTV system to the specification of South Yorkshire Police will be fitted, maintained and in use at all times whilst the premises are open. The CCTV images will be stored for 31 days and police and authorised officers of the council will be given access to images for purposs in connection with the prevention and detection of crime and disorder. CCTV footage shall be downloaded and provided to South Yorkshire Police on request. Members of the management team will be trained in the use of the system. A copy of the specification dated January 2010 will be available at all times for inspection of the police and authorised officers. 10. The premsies are to acquire a suitable number of radio set(s) for that premise and take the CCRAC Radio Scheme into use at all times when trading, and continue to be a user whilst this system / facility is in use in Sheffield. 11. Staff training must include the operation of a challenge 21 scheme, how to maintain refusals records, how to report and respond to incidents, how to manage confrontation, and the knowledge of the licensing conditions and the policies operating at the premises. Staff training records including refresher training must be maintained to the satisfaction of the licensing authority and be made available to the officers of the responsible authorities on request. 12. All persons who have left the premises for whatever reason and who seek re-entry must go through the security and admissions procedure, expect for those using the smoking area. The people using the smoking area will be re-searched on a risk assessed basis. 13. The premises shall have in force and operate at all time the premises are open to the public a policy to prevent unlawful drugs, weapons and articles that maybe used a weapon being brought into the premises. This shall include as a minimum the use of a metal detecting device and shall apply to persons entering or re-entering the premises. The policy shall be recorded in writing and all staff involved in the implementation shall be made aware of the policy. 14. Every Friday, Saturday and bank holiday weekend there will be at least two members of SIA registered doorstaff from 20:00hrs increasing a minimum of three at 2300hrs and six from 00:00hrs until the terminal hour. From 20:00hrs Sunday to Thursday there shall be at least two members of SIA registered doorstaff on duty until the terminal hour. 15. After 20:00hrs on a Friday, Saturday and Bank Holiday Weekend drinks shall not be taken into the designated smoking area, and there shall be at least one member of SIA registered door staff to monitor this area. 16. A minimum of one designated member of doorstaff must be assigned to verify the age of all persons accessing the premises. 17. A copy of the South Yorkshire Police Violent incident protocol shall be displayed in the premises in view of all staff. This will form part of staff training and records will be kept to reflect this. 18. Polycarbonate drinking vessels shall be available and used on a management risk assessed basis and through negotiation of South Yorkshire Police advice. 19. The Premises Licence Holder shall prominently A4 notices in all external areas reminding patrons to be quiet whilst using the facilities provided and that they are in close proximity to residential premises. 20. The DPS for these premises must be a registered member of Sheffield license watch, annual membership and meeting attendance shall be maintained.
1. No licensable activities shall be carried out on the premises until a Risk Assessment and a set of plans has been submitted to and approved by the Health Protection Service.
