Additional Conditions

1. Persons under the age of 18 are not permitted. 2. At the point of entry to the premises, a Challenge 25 proof of age scheme must be operated in compliance with the multi agency guidance issued by the Safeguarding Children Board, Police and Trading Standards. 3. All employees, volunteers or entertainers must be aged 18 years or over. A record of each employees name, address, national insurance number, photograph and date of birth will be retained and these details must be checked and verified by the premises manager or premises licence holder, against other recognised forms of identity (passport; photo-driving licence). These records must be retained at the premises for a minimum of 12 months from the date of employment and made available to officers of Responsible Authorities on request. 4. Signage must be displayed at points of entry that persons under the age of 18 are prohibited and that the Challenge 25 scheme operates. 5. Persons re-entering the premises must be subject to the same security and identity checks as customers first entering the premises at any given period of trade. 6. Persons using VIP/Guest passes to access the premises must be subject to the same security and age verification checks as non concessionary customers. 7. The number of persons admitted to the premises will be determined by a risk assessment and agreed with South Yorkshire Fire and Rescue. This number will be policed by a clicker system. 8. S.I.A. registered door staff will in place from 23:00 hours until the premises close at a ratio agreed with the Local Authority and the Police. A female supervisor will be available if a search of females needs to be carried out. 9. Staff will be trained to identify any customer causing concern or drinking excessively and management will patrol the floor area to prevent any anti social behaviour. 10. All inclusive nights or other similar drinks promotions will not be permitted. 11. No persons will be allowed to take open containers of drink from the premises. 12. No persons carrying sealed or opened bottles or glasses will be admitted to the premises at any time. 13. An anti drugs policy agreed with South Yorkshire Police and the Local Authority will be in force and a zero tolerance towards drugs adopted. 14. Persons who appear drunk or under the influence of drugs will not be admitted at any time. Any customers who appear to be drunk or under the influence of drugs whilst on the premises will not be allowed to remain and appropriate steps will be taken to ensure their departure. 15. The behaviour of both staff and performers will be strictly monitored and any unruly and unacceptable behaviour will not be tolerated 16. A log will be maintained of all incidents/accidents which occur on the premises. 17. All instances of crime and disorder shall be reported to South Yorkshire Police and an incident log will be maintained. 18. An adequate and appropriate supply of first aid equipment will be available on the premises. 19. All recommendations of South Yorkshire Fire and Rescue will be promptly acted upon. 20. When disabled people are present, adequate arrangements will exist to ensure their safe evacuation from the premises. 21. A permanently fixed residual current device (RCD) must protect the electrical supply serving all amplified music equipment for the purpose of live music and DJ. 22. The applicant or any person purchasing these premises must liaise with and fully comply with the reasonable requirements of the responsible authority towards public safety during the fit out of the premises. 23. No licensable activities will take place under the authority of this licence until the authority has been notified that the premises are suitable. Any reasonable requirements of the public safety authority will be complied with in order to ensure that the premises are suitable for their intended use. The licensee will give the responsible authorities a minimum of 14 days notice of the intended opening date of the premises. 24. A Building Regulation Completion Certificate shall be submitted to the public safety and the Licensing Authorities before the premises is used for any licensable activities. 25. Amplified sound or live music shall only be played within the building in such a way that noise breakout to the street does not exceed. (i) background noise levels by more than 3 dBA when measured as a 15 minute LAeq. (ii) Any octave band centre frequency by more than 3db when measured as a 15 minute Leq. 26. No amplified sound shall be played except through an in-house amplified sound system fitted with a sound limiter. The setting of which shall have received the prior written approval of the Environmental Protection Service. 27. All doors and windows shall remain closed, save for access and egress when regulated entertainment is present in the premises. 28. The DPS or Designated member of staff must take a pro-active approach to noise control, regularly checking outside the premises to ensure that noise is kept to a reasonable level from patrons using any external area or waiting to access the premises and manage noise levels internally so as not to cause a nuisance. 29. No more than 12 people will be outside the premises at any one time for the purpose of smoking and this will be monitored by the registered door staff. 30. A taxi scheme will be in force and a ring back system will be used. 31. Staff training will be provided in relation to alcohol, records will be kept and maintained to reflect this. 32. A colour CCTV System to the specification of South Yorkshire Police, will be fitted, maintained and in use at all times the premises are open, the CCTV images will be stored for 28 days and Police and Authorised Officers of the Council will be given access to images for purposes in connection with the prevention and detection of crime and disorder.
