ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
1. A CCTV system to the specification of South Yorkshire Police will be fitted, maintained and in use at all times when the premises are open. CCTV images will be stored for 31 days and Police and Authorised Officers of the Council will be given access to images for purposes in connection with the prevention and protection of crime and disorder. CCTV footage shall be downloaded and provided to South Yorkshire Police on request. Members of the management team will be trained in the use of the system.
2. Alcoholic drinks may not be removed from the premises in open containers save for consumption in external areas provided for that purpose and no customers carrying open alcoholic drinks upon entry shall be admitted to the premises.
3. The use of door staff will be risk assessed on an ongoing basis by the licence holder or premise supervisor. A written risk assessment carried out by the premises. A copy of the risk assessment will be kept on the premises for a minimum of 6 months. The risk assessment will be made available immediately on request by the police.
4. Any persons exercising security (as defined by Paragraph 21a of Schedule 2 of the Private Security Act 2001) shall be licensed by the Security Industry Authority. Such persons will be employed at the premises at the discretion of the DPS/ holder of the Premises Licence.
5. There shall be a zero tolerance drugs policy at the premises and there shall be a suitably secure ‘drop box’ for any illegal substances or items confiscated from customers. Seized items will be handed to South Yorkshire Police.
6. All staff authorised to sell alcohol shall be trained in:
a. Relevant age restrictions in respect of products
b. Preventing underage sales
c. Preventing proxy sales
d. Maintaining the refusals log
e. Recognising signs of drunkenness and vulnerability
f. How overservice of alcohol impacts upon the four objectives of the Licensing Act 2003
g. How to refuse service
h. The premises’ duty of care, understanding and dealing with situations involving vulnerable people, and incidents of harassment; and how to report issues of modern slavery and trafficking.
i. ‘Ask for Angela’
j. Action to be taken in the event of an emergency, including the preservation of a crime scene and reporting an incident to the emergency services
k. The conditions in force under this licence.

7. Documented records of staff training shall be kept for each member of staff. Training shall be regularly refreshed and at no greater than 12 monthly intervals. Training records shall be made available for inspection upon request by a police officer or an authorised officer of Sheffield City Council.
8. The PLH/DPS will ensure that an Incident Report Register is maintained on the premises to record incidents such as anti-social behaviour, admissions refusals and ejections from the premises.
9. The Incident Report Register will contain consecutively numbered pages, the date time and location of the incident, details of the nature of the incident, the names and registration numbers of any door staff involved or to... whom the incident was reported, the names and personal licence numbers (if any) of any other staff involved or to whom the incident was reported, the names and numbers of any police officers attending, the police incident and / or crime number, names and addresses of any witnesses and confirmation of whether there is CCTV footage of the incident.
10. The Incident Report Register will be produced for inspection immediately on the request of an authorised officer.
11. Written records of all accidents and safety incidents involving members of the public will be kept. These will be made available at the request of an authorised officer.
12. A written spillage policy will be kept, ensuring spillages are dealt with in a timely and safe manner.
13. Members of the public will be prevented from accessing hot food and drink preparation areas to prevent risk of scalds or burns to them. d) The prevention of public nuisance
14. No amplified sound shall be played within the building except through an in house amplified sound system fitted with a sound limiter, the design and settings of which shall have received the prior written approval of the Environmental Protection Service.
15. An outdoor 'Noise Management Plan' shall be submitted and approved in writing by the EPS. The approved NMP shall include details of suitable arrangements to minimise noise breakout of the building, management of people outside the venue, detail measures for managing arrivals and departures, including a solution to manage smokers or vapers. A copy of the approved NMP shall be retained on site and be made available upon request by the EPS.
16. The DPS shall prominently display notices at all exits and in outside areas reminding patrons to leave the premises in a quiet and orderly fashion and to respect the local neighbours' needs and when using outside areas.
17. Bottles and waste shall not be placed in any external receptacle Monday to Saturday between 23:00 and 07:00 hours, and between the hours of 23:00 to 09:00 on Sundays and Public Holidays.
18. No deliveries to the premises shall take place between 23:00 and 07:00 hours.
19. Patrons shall not use the external areas other than for smoking after 23:00hrs.
20. The Challenge 25 scheme must be operated to ensure that any person who appears to be under the age of 25 shall provide documented proof that he/she is over 18 years of age. Proof of age shall only comprise a passport, photo card driving licence, an EU/EEA national ID card or similar document, an HM Forces warrant card, a card bearing the PASS hologram, or any electronic or biometric age verification technology approved by the licensing authority.
21. Prominent signage must be displayed, at any point of sale and at the entrance to the premises, that the Challenge 25 scheme is in operation.
Agreed EPS Conditions - 24.05.24
22. No loudspeakers shall be located externally nor directed to broadcast sound outside the building at any time.
Agreed Police Conditions - 28.05.24
23. The use of glass alternative drinking vessels shall be utilised in accordance with a written risk assessment. The written assessment to be retained on the premises at all times and reviewed every 6 months.
24. Events will be held on a pre-booked basis only and will not exceed a capacity of 499 persons.
25. Persons under the age of 18 must be accompanied and supervised by an adult and are not permitted after 21:00 hours. Unless accompanied by a responsible adult and as part of a prearranged, family type event.
26. The Designated Premises Supervisor, or other such responsible person, should be assigned to act as the Co-ordinator for safeguarding systems at the premises. This person must act in compliance with the guidance and training provided by Sheffield Children’s Safeguarding Partnership.
Agreed Health & Safety Conditions - 28.05.24
27. There shall be no transportation of hot food and drinks using the public staircase between the ground floor and first floor (whilst open to members of the public).
28. A Building Regulation Completion Certificate shall be submitted to the responsible authority for public safety within a reasonable time period after the premises has opened for business.
29. For licensable activities & events over 250 persons, additional temporary toilet facilities will be provided in accordance with BS 6465-1:2006+A1:2009 (as amended).
