Annex 2 - Conditions consistent with the operating schedule
1. There shall be no adult entertainment or services, activities, other entertainment or matters ancillary to the use of the premises that may give rise to concern in respect of children.
2. All doors and windows shall be closed, save for access and egress, when amplified sound is present in the premises.
3. Music should only be played within the building such that:
(i) Noise breakout from the building to the site boundary should not exceed the ambient* noise levels by more than 3 dB(A) when measured as a 15 minute
(ii) Noise breakout from the building to the site boundary should not exceed the ambient* noise levels in any octave band centre frequency by more than 3dB when measured as a 15 minute
*Where ambient noise level is the LAeq, 15mins in the absence of the specific noise source (breakout from the premise).
4. The DPS or designated member of staff must take a proactive approach to noise control, checking outside the premises to ensure that noise is kept to a reasonable level from patrons using external area(s) and access/egress.
5. The premises licence holder shall prominently display A5 notices on all exits reminding patrons to leave the premises in a quiet and orderly fashion to respect the local neighbour's needs.
6. It is considered that for the nature of the operation door supervisors will not be required however, a risk assessment will be undertaken should unusual events take place and if they consider it appropriate will employ the same.
7. Events will only be held at the licensed premises following a booking procedure, whereby the Premises Licence Holder / DPS will ensure that if a function is to be booked then full details of the name, address and telephone number of the person to hold the event will be obtained and made available to the Police upon request.
8. A booking form must be used for all events and private functions. The form must set out the booking conditions for the customer and include the signature and printed name of the venue manager authorising the booking. The form must also include the name, contact details and signature of the customer who is responsible for making the booking. Such records will be retained for 6 months and made available for inspection of the authorities.
9. Persons under the age of 18 years are not permitted to make bookings. Age/identity checks will be undertaken at the time of booking and written record of these will be made.
10. The Designated Premises Supervisor, or other such responsible person, will be assigned to the role of Safeguarding Coordinator. This person should act with reference to the safeguarding training and guidance provided by the Sheffield Safeguarding Children Board.
11. The management will undertake training for all staff so that they are aware of the premise licence so that they are aware of the licensing objectives.
12. A CCTV system will be installed to monitor entrances, exits and other parts of the premises.
13. Deliveries of goods necessary for the operation of the business will be carried out at such a time or in such a manner as to prevent nuisance and disturbance to nearby residents.
14. The movement of bins and rubbish outside the premises will be kept to a minimum after 22:00 hours.
15. Any lighting on or outside the premises will be positioned and screened in such a way so as to not cause a disturbance to nearby residents.
16. Adequate waste disposal receptacles for use by customers will be provided in the local vicinity.
17. "Challenge 25" signage is to be erected to encourage anyone who is over 18 but looks under 25 to carry acceptable ID (a card bearing the PASS hologram, a photographic driving licence or passport) if they wish to buy alcohol. Staff will be trained about the requirement for a persons' ID.
18. A log book is to be maintained and kept on the premises at all times.
